Antimalarials are drugs which are used for prophylaxis, treatment & prevention of Malaria. They are used for treatment of Malaria in individuals with suspected or confirmed infection and for prevention of infection in individuals visiting a malaria-endemic region who have no immunity ( prophylaxis) .
Dihydroartemisinin 128mg + Piperaquine Phosphate 1280mg + Trimethroprim 360mg Oral Suspension is an oral fixed-dose artemisinin-based Combination antimalarial and antiprotozoal agent. This combination therapy is used for treatment of all forms of malaria, including those caused by multi drug-resistant strains. Dihydroartemisinin + Piperaquine Phosphate + Trimethoprim is indicated for the treatment of various types of malaria including chloroquine-resistant P. falciparum and P. vivax.

Dihydroartemisinin is the active metabolite to which artemesinin is reduced. It is the most effective artemesinin compound and the least stable. It has a strong blood schizonticidal action and reduces gametocyte transmission. It is used for therapeutic treatment of cases of resistant and uncomplicated P. falciparum. 4 mg/ kg doses are recommended on the first day of therapy followed by 2 mg/ kg for 6 days. As with artesunate, no side effects to treatment have thus far been recorded.
Pipreraquine is an antimalarial compound belonging to the 4-aminoquinolines. It is a highly liphophilic. Piperaquine Phosphate ( PQP) interferes with physiological function of the food vacuole membrane of the trophozoites leading to autophagocytosis of the parasites. It has no marked effect on the ring forms, immature or mature schizonts and the male of female gametocytes.
Trimethoprim acts by interfering with the action of bacterial dihydrofolate reductase, inhibiting synthesis of tetrahydrofolic acid. Tetrahydrofolic acid is an essential precursor in the de novo synthesis of the intermediate Thymidine monophosphate ( dTMP) , precursor of DNA metabolite Thymidine triphosphate. Bacteria are unable to take up folic acid from the environment ( i.e. the infection host) and are thus dependent on their own de novo synthesis. Inhibition of the enzyme starves the bacteria of nucleotides necessary for DNA replication causing, in certain circumstances, cell lethality due to thymineless death.
